2. Complete concussion baseline testing by reserving a testing appointment with BHS
3. Take physical forms to your doctor after printing out from Final Forms: Preparticipation Physical Eval Form (Stays with Doctor), Physical Exam (Stays with Doctor), and Medical Elgibility Form (Doctor completes and you Upload into Final Forms). 
4. Pay for ASB Card ($50/year) and Participation Fee ($250/sport) online or by check/cash

BISD athletics has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking that ALL parents or athletes use FinalForms.

All athletes must also have a current (no older than 24 months) baseline concussion test on file in the office before turning out for a sport.

Each Athlete Must Must Pay The Following Fees:

• ASB Card: Online payment for the ASB card is $50 for the school year
• Participation Fee: $250/sport participation fee

Fees are payable as soon as an athlete makes a roster and prior to the first competition.  These fees may be paid through the online payment site or by check or cash at our attendance office.
